We will need the following Information to file test claims:
- Patient name
- Date of birth
- Call back phone numbers
- Physician name and phone number
- Name of primary insurance
- Member ID or contract number
- Group number
- Rx bin number
- PCN number
- Name of policy holder
- (include same info if secondary plan available)
- List of needed medications

h.e.a.r.t. + Pharmacy Savings Program
Southern Pharmaceutical Services is a participating provider in the h.e.a.r.t.+ Pharmacy Savings Program.
