We will need the following Information to file test claims:

  • Patient name
  • Date of birth
  • Call back phone numbers
  • Physician name and phone number
  • Name of primary insurance
  • Member ID or contract number
  • Group number
  • Rx bin number
  • PCN number
  • Name of policy holder
  • (include same info if secondary plan available)
  • List of needed medications

h.e.a.r.t. + Pharmacy Savings Program

Southern Pharmaceutical Services is a participating provider in the h.e.a.r.t.+ Pharmacy Savings Program. The h.e.a.r.t. + Program helps cash-paying and insured patients save money on Ferring Pharmaceuticals’ fertility medications.
